[Muscle] GemPC pinpad reader issue on Mac

Hi,

FYI: it seems on Mac OS X, the GemPC pinpad reader
must be inserted when the pcscd starts. So if
you remove the reader and plug it in again, it's
no longer visible with SCardListReaders().

It seems to works fine if we remove the ccid-1.3.8
driver (and restart pcscd so it uses Apple's CCID
driver).

Little Q: how to pcscd decide which CCID to take?
Is it randomly, or is there some priority mechanism?
