[Muscle] CCID driver for Mac OS X

Hello,

Mac OS X Tiger (10.4) uses an old pcsc-lite with two problems:
- a missing call to IFDHSetProtocolParameters() so the reader is not
configured correctly regarding to the card ATR
- a wrong API for SCardControl() so you can't use a pin pad reader or
any other features provided by SCardControl()

To use my CCID driver on Mac OS X Tiger you can use the patch I provide in [1].

Mac OS X Leopard (10.5) provides pcsc-lite 1.4.0 so the patch is not
needed. I do not plan to include the patch in the driver since the
patch will be obsolete when Leopard is released (October 2007) and
will even be problematic.
